Billing
If you have a question about your statement, please call the office between 9:00 am and 5:00 pm and ask for our billing department. We will make every effort to explain your bill to you and assist you in understanding insurance plan coverage. In some cases, we may ask you to contact your insurance company if there is a problem with payment. We can also answer your billing questions by emailing us your question at: kdavis@mibsi.com. Please provide your name, account number and date of service along with your question for us. All email questions will be answered within 1 business day by a return email to the address you have sent to us unless otherwise notified within your original email message.
We are participants in many insurance companies and have provided a list of some of the more common plans that we see in the office. If you do not see your insurance on our list, please call the office and ask us if we accept that plan or contact your member services representative at your insurance company and ask if we are on the provider list. We will assist you by filing your claims for you with your insurance company. If you are required to pay an office co-pay, deductible or a percentage of the office fees, we ask that you do so when you are in the office for your appointment. We accept cash, checks, Mastercard and Visa.
Please notify our office (Ypsilanti, Chelsea, Brighton) if you have a change of address or insurance as soon as possible so that we can update our records. Always bring your insurance card and any other pertinent insurance information with you to each appointment.
Please note: Managed care companies or HMO’s, require that you receive a referral authorization prior to your visits in our office. It is important to bring a copy of your written referral notice from your insurance company/primary care provider to your appointment.

1. Traditional Blue Cross/Blue Shield, Blue Preferred PPO, Blue Preferred Plus PPO, Blue Choice, Community Blue, FEP Blue Cross
2. Blue Care Network
3. Medicare, Medicare Plus Blue, BCN Advantage, and Hap Senior Plus
4. Medicaid (including Midwest Health Plan)
5. HAP HMO
6. Priority Health HMO and PPO
7. Auto and Worker's Compensation with prior authorization
8. Commercial Insurances: We will submit any charges to your insurance company directly for reimbursement. You do not have to prepay your visit. You will be responsible for any non-covered amounts, co-pays or deductibles.
Please check with your insurance to verify your coverage prior to scheduling your appointment. Please be aware that your insurance coverage is a contract between you and your insurance provider. We are not a party to that contract.
